CLEARFIELD – Preliminary hearings have been scheduled in Clearfield County for 9:30 a.m. Nov. 10 for the two inmates who have been charged with escaping from the Quehanna Boot Camp in Karthaus on Sunday, according to court documents.

Nicholas Guido, 30, and Trent Robert Maffei, 20, have both been charged with escape, F3, by the Clearfield-based state police. Both are now incarcerated at the Clearfield County Jail in lieu of $100,000 bail.

According to the affidavit of probable cause, at approximately 12:50 p.m. Sunday, Guido and Maffei escaped from the State Correctional Institution at the Quehanna Boot Camp in Karthaus.

Guido and Maffei were both inmates at the boot camp. Guido and Maffei were ordered to the Quehanna Boot Camp by the Courts of Common Pleas in Allegheny and Juniata counties, respectively.

Guido and Maffei were each serving 24-month State Intermediate Punishment (SIP) sentences. Guido was sentenced for a burglary with no person present in Allegheny County and Maffei for possession with intent to deliver in Juniata County.

Guido and Maffei were with other inmates outside within the boot camp property on Sunday. Both were allegedly observed running to a black Sedan that parked off of Route 1011. Both Guido and Maffei entered the vehicle, which then fled north.

State police allegedly believed Guido and Maffei were in the Harrisburg area following their escape and with Blondena Marie Thompson, 19, of Hummelstown. Thompson has been charged with two counts of hindering apprehension or prosecution by Newport-based state police and is incarcerated in the Perry County Prison in lieu of $20,000 bail.

State police told the Carlisle Sentinel that Thompson allegedly allowed Guido and Maffei to use a phone, as well as provided them with transportation. All three were allegedly located by authorities in Thompson’s vehicle on Monday.

HARRISBURG — Attorney General Kathleen G. Kane today announced criminal charges have been filed in Blair County against 30 suspected drug dealers.

The charges are the culmination of multiple investigations conducted by the Office of Attorney General’s Bureau of Narcotics Investigation and the Altoona Police Department.

The investigations, which began in September of 2014, primarily focused on individuals distributing heroin, prescription medication, marijuana and cocaine in Altoona and the surrounding municipalities. The alleged drug dealers were mainly street- to mid-level dealers, investigators reported.

The investigations consisted of approximately 52 controlled purchases, which involved undercover investigators and confidential informants. Of those charged, 10 cases involved heroin, nine involved prescription medications, three concerned cocaine or crack and eight concerned marijuana, according to investigators.

Controlled purchases took place in public streets, parking lots and inside of suspects’ residences. Information gathered from these investigations, including sources of supply, is used to target mid- to upper-level drug dealers. More arrests are anticipated as the investigations continue.

The suspects charged were expected to be apprehended during a collaborative roundup that included agents from the Office of Attorney General, officers from the Altoona Police Department and several other law enforcement agencies.

Other agencies participating in the roundup include the Blair County district attorney’s office, the Blair County Drug Task Force, the Allegheny Township Police Department, the Blair Township Police Department, the Logan Township Police Department, the Hollidaysburg Police Department, the Greenfield Township Police Department, the Tyrone Police Department, the Duncansville Police Department and the Williamsburg Police Department.

The defendants are expected to be preliminarily arraigned before Blair County Magisterial District Judge Paula Aigner. Each defendant will be prosecuted by the office of Blair County District Attorney Richard Consiglio.

Nurse Accused of Taking Drugs Intended for Patient

DUBOIS – A Grassflat woman has been charged with taking drugs intended for a patient at a local hospital.

Shauna Lynn Burnett, 34, 1178 Cooper Rd., Grassflat, was charged with acquiring or obtaining possession of a controlled substance by misrepresentation, intentional possession of a controlled substance by person not registered and furnishing false/fraudulent material information by the Pennsylvania Attorney General’s office on Oct. 30.

According to the affidavit of probable cause, Burnett is a registered nurse who worked for Fresenius Medical Care and was periodically assigned to the dialysis unit of Penn Highlands DuBois. In January the report of an automated narcotic dispensing instrument known as AcuDose showed that Burnett was using the system to obtain hydromorphone, morphine, lorazepam and oxycodone.

The nurses in her unit are supposed to obtain any pain medications for patients from a nurse in the Progressive Care Unit who would use their access to the system to retrieve the medication.

Burnett was counseled and instructed to follow the established procedure.

In February the director of the Progressive Care Unit became aware Burnett had obtained controlled substances from AcuDose units outside the dialysis unit.

Burnett’s supervisor at Fresenius Medical Care was contacted and asked to remove Burnett from the hospital. She was drug tested prior to leaving the hospital but the results were negative.

Burnett told a supervisor she “had a problem” and admitted she takes pain medications intended for other people.

In August a registered nurse who was also a dialysis patient in February was interviewed by investigators. He explained if he was in pain during or after his treatment he would take Tylenol. After his treatment on Feb. 18, he requested Tylenol from the nurse in anticipation of a headache. She refused his request because his medical record authored by Burnett indicated he had pain assessed at level “6” and had already been given hydromorphone by Burnett. He stated he did not receive the drug that day and he had not been asked by anyone to evaluate his level of pain.

Records from the AcuDose indicate on that date at 10:33 a.m. Burnett removed hydromorphone for the victim.

Burnett’s preliminary hearing is scheduled for Dec. 18 before District Judge Patrick Ford at his office at 9:30 a.m.

Fugitive of the Week: Joshua Cribbs (Provided photo)

CLEARFIELD – District Attorney William A. Shaw Jr. has announced Clearfield County’s Fugitive of the Week for the week of Nov. 9.

Shaw identified the fugitive as Joshua Cribbs, 33, of Church Street, Dubois. Cribbs is wanted for failure to appear at Criminal Call on Oct. 29.

On April 27 Officer David Tracy of the Dubois City police filed a criminal complaint. He charged Cribbs with home improvement fraud, F2; theft by deception, F3; and theft by failure to make required disposition of funds, F3.

According to the affidavit of probable cause, on June 4, 2014, Tracy contacted the victim who alleged that Cribbs had received advance payment for a home improvement project. However, the victim said that Cribbs had allegedly failed to perform the work.

The victim told police that Cribbs had allegedly received a down payment for the purchase of materials to complete the home improvement project. Cribbs allegedly failed to purchase any of the materials for the project.

Numerous attempts to contact Cribbs regarding the project resulted in no response, the victim alleged to police. Despite further efforts to resolve the matter, Cribbs allegedly continued to neither provide the victim with payments nor complete the project.

A preliminary hearing was scheduled for May 29, at which time all charges were held to court. On Oct. 29, Cribbs failed to appear at Criminal Call, and a bench warrant was issued for his arrest.

DUBOIS – A DuBois woman faces felony charges for giving beer to minors, exposing herself and then offering to pay one of them to withhold information from police.

Stephanie Marie Fultz, 31, 42 W. Scribner Ave., DuBois, was charged by DuBois City police with a felony count of intimidation of witness, a felony count of corruption of minors, indecent exposure, two misdemeanor counts of corruption of minors and three counts of sell or furnishing malt or brewed beverages to minors in relation to an incident at her apartment on Aug. 7.

The charges were filed Oct. 26 through District Judge Patrick Ford’s office. Her preliminary hearing is scheduled before Ford on Dec. 4 at 9:30 a.m.

According to the affidavit of probable cause, police were investigating a report about a theft by Fultz when they arrived at her apartment Aug. 7. Her apartment door was open and after announcing themselves as police officers, they saw Fultz and a male appear out of a room to the left of the stairs. Fultz emitted a strong odor of an alcoholic beverage and was holding a can of beer.

Upon entering the officers also made contact with two other juvenile males. A case of beer was on the living room floor and multiple empty cans were on the floor, tables and in the kitchen sink. Both males were asked how much they had to drink. All three males admitted they had consumed some of the beer.

One of the males provided a written statement to police Aug. 9 advising Fultz had provided the alcohol for them. He said she called him into the restroom at one point during the night. When he opened the door, she had her pants completely down, fully exposing herself to him. He left the bathroom and she approached him saying “no, it’s OK.” She then hugged him and stated “I don’t care if you’re 15, just don’t get me for rape.”

The day following the incident, he received numerous calls from Fultz. When he answered she offered to pay him two different amounts of money, $400 and then $800 to withhold information from police. She wanted him to say he was already drunk when he arrived at her apartment.

Another of the males provided a statement saying that she purchased and supplied the beer. He said he also knew that Fultz had offered the other male money to withhold information.

CLEARFIELD – A Clearfield man accused of assaulting a man inside a local bar waived his right to a preliminary hearing Wednesday during Centralized Court.

Robert Paul Burkett, 29, 710 Weaver St., Clearfield, is charged with aggravated assault, simple assault, recklessly endangering another person, disorderly conduct-engage in fighting and harassment as a result of an incident on Nov. 4 at Mary’s Bar in Clearfield Borough. His bail was set at $25,000 unsecured.

According to the affidavit of probable cause, at 1:43 a.m. police were called to a fight in progress at the bar. When the officers arrived they found a man lying on the floor with a large amount of blood flowing from his nose/face area and a woman providing medical treatment to this victim.

An employee of the establishment said the victim and Burkett had a physical altercation, which led to the victim lying on the floor. At that point, Burkett allegedly began kicking the victim three or four times about the head, face and nose causing blunt force trauma to those areas. The victim’s teeth were forced through his lips causing open wounds.

In addition to the large amount of blood on the floor around the victim, there was a lot of blood coming from the victim’s nose.

Burkett had fled the establishment once he was told the police had been called.

Clearfield EMS responded and medical personnel treated the victim before taking him to the hospital. He was later flown by helicopter to the Altoona Hospital Trauma Center for an evaluation and treatment.

The officers searched the area for Burkett, who was on foot and traveling south on South Third Street. Because they were unable to find him, they requested assistance from Lawrence Township police officers. They went to Burkett’s home and knocked on the front door, but no one came to the door.

CLEARFIELD – The Lawrence Township Police Department is investigating an alleged “push out” retail theft incident that occurred at the Wal-Mart Supercenter in Clearfield.

According to police, the alleged theft occurred between 3:50 a.m. and 4:10 a.m. July 12. Asset protection personnel told police that an assistant manager allegedly found a Caucasian male walking out of the garden center’s fire door with a tote full of merchandise at 4:10 a.m.

Asset protection personnel told police that after reviewing surveillance video, they allegedly watched the male put tablets, laptops and routers in a large storage tote that was being pushed in a cart. He also allegedly took a computer and monitor before exiting the fire door in the garden center without paying for the merchandise.

Police said that the video showed a dark-colored sports utility vehicle with an unknown driver approaching the garden center fire door to pick up the male. The male allegedly fled in the vehicle and left an empty cart in the Wal-Mart parking lot.

The total value of the stolen items is $1,985.84, police said. Asset protection personnel told police that they contacted the Dubois Wal-Mart store, which confirmed the male appeared to be the same suspect in recent retail thefts there.
